The Greater Severna Park Athletic Association is committed to provide information and education on concussions and concussion awareness for our athletes, parents and coaches.
GENERAL INFORMATION
Please see the Maryland Youth Concussion Legislation signed into Law by Governor Martin O'Malley on May 19, 2011 regarding current concussion legislation.

To be in compliance with the Maryland Youth Concussion Legislation Law each youth organization within Anne Arundel County is required to complete and keep on file a Concussion Awareness Form. This form needs to be completed by all participants within our athletic program and kept on file. The GSPAA (Green Hornets) will have this form as part of our on-line registration process that is to be read and reviewed by the athlete and parents, electronically approved and will be filed as part of the registration sign-up process. There will be a mandatory check-box for acknowledging the form has been read and approved in the on-line sign up process moving forward.

RETURN TO PLAY FORM: (Mandatory)
For ANY athlete that has experienced a concussion, A RETURN TO PLAY FORM must be completed and given to the head coach, medically clearing the athlete to RETURN TO PLAY.
